1. Eligibility
- The scheme is open to all residents of Maharashtra, particularly families from economically weaker sections.
- Individuals must submit valid documents for verification before receiving a health card.
2. Scope of Benefits
Cardholders are entitled to:
- Discounts ranging from 10% to 50% on consultation fees at partnered hospitals, clinics, pathology labs, and medical stores.
- Access to regular and emergency healthcare facilities under listed service providers.
- Benefits may vary depending on the service provider and region.
3. Card Usage
- The health card is non-transferable and must be used only by the registered cardholder or their listed family members.
- Misuse or duplication may result in cancellation without refund or notice.
4. Employment Services
Our foundation may also provide employment opportunities to eligible and educated unemployed individuals, as part of our social commitment.
5. Responsibilities of the Cardholder
- Ensure that your information is up-to-date.
- Present your health card at the time of availing services.
- Adhere to the rules of each partnered facility.
6. Limitation of Liability
- We are not responsible for the actions or services of third-party hospitals, clinics, or pharmacies.
- We will not be liable for any indirect or consequential damages resulting from participation in the scheme.
7. Termination
We reserve the right to modify, suspend, or terminate the Health Card Scheme at any time, without prior notice.
